Why does the heart need a ‘restart’?

Our heart normally beats at a steady rhythm – like a well-timed motor. This rhythm is controlled by electrical impulses that ensure that the heart muscle contracts in a coordinated manner and pumps blood through the body.

In the event of sudden cardiac arrest, this electrical system becomes disrupted. The heart then beats either much too fast, chaotically or not effectively at all. In this state, it can no longer pump blood to the body – vital organs such as the brain are no longer supplied with oxygen.

What does an AED do?
An AED (automated external defibrillator) is a device that helps in precisely this situation. After the electrodes are placed, it automatically analyses the heart rhythm and decides whether an electrical impulse is necessary. If necessary, the user is prompted by a voice command to press the shock button.

The electrical impulse is not a ‘restart’ in the sense of a switched-off heart, but rather a reset of the chaotic heart rhythm.

11 / Paragraph 1 of the Medical Devices Operator Ordinance:

  • The operator may only allow a product listed in Annex 1 to be operated and used if the manufacturer or an authorised person acting in agreement with the manufacturer has
  1. has subjected this product to a functional test at the place of operation and
  2. a person commissioned by the operator has instructed them in the proper handling, use and operation of the product, interconnected products and objects associated with the products on the basis of the instructions for use and the accompanying safety-related information and maintenance instructions.

 

An STK (safety inspection) is not necessary for aluderm AEDs (applies to purchases made after 1 January 2025). The devices perform regular self-tests and store the results. A visual inspection must be carried out by the operator at regular intervals.

Notwithstanding paragraph 1, safety checks may be omitted for automatic external defibrillators intended for use by laypersons if the automatic external defibrillator is self-testing and regular visual inspections are carried out by the operator. From 1 January 2027, newly commissioned automatic external defibrillators intended for use by laypersons shall also document the results of the self-tests and automatically transmit them to the operator on a regular basis via remote monitoring. In the cases referred to in sentence 2, notwithstanding sentence 1, the visual inspection by the operator may be carried out on an ad hoc basis, depending on the results of the remote monitoring. The operator must make the data on the functionality, location and public accessibility of the automatic external defibrillator publicly available through the manufacturer. The operator must ensure that the functionality of the automatic external defibrillator is restored immediately after a failed self-test.

 

 

Anyone can use an AED, even without prior medical training. The device provides clear verbal instructions (A1 + A2) or graphic instructions (A2) and automatically analyses the heart rhythm, so the user only has to follow the instructions.

 

Yes, AEDs are very safe. They automatically analyse the heart rhythm after the electrodes are placed and only deliver a shock if necessary. Incorrect operation that could harm the patient is virtually impossible.

 

Many cities, municipalities and organisations list AED locations online. There are also apps that show the nearest AEDs. It is a good idea to find out about locations in the area in advance.

 

As a first responder, you are legally protected. The most important thing is to remain calm, follow the AED instructions and alert the emergency services at the same time.
Section 323c of the Criminal Code (StGB) obliges people to provide assistance in emergencies, as far as this is reasonable. At the same time, it protects helpers from being held liable for damage that is unavoidable and occurs in the course of emergency assistance.

No one will be prosecuted for mistakes made while trying to save a life. On the contrary, failure to help can be punishable by law. An AED helps you to provide first aid as effectively and safely as possible by guiding you step by step through its use.

No, as a private individual, you are generally not subject to any legal operator obligations such as STK, training or documentation – as long as the AED is used exclusively for private purposes.
